An example of this would be if a very large clinical trial compared thrombolytic therapy (clot dissolving medication) to direct percutaneous coronary intervention (balloon dilation with possible stent placement in a heart artery) and did not treat the mechanical intervention (balloon dilation) group with any antiplatelet therapy (antiplatelet inhibits clot formation).  The outcome of the mechanical revascularization strategy (balloon dilation treatment) in this trial would be inferior to thrombolytic therapy (clot dissolving medication) because balloon dilatation outcome is dependent on having antiplatelet therapy present.
